Thank you for your interest in the Fossil Creek After School Program. Below are all of the forms needed to register your child in the program. A registration packet may also be picked up at the center. Before we can enroll your child we will need:

 

-A completed registration packet (click and print ALL forms found below).

-All enrollment fees must be paid at time of enrollment.

               - $50.00 Registration Fee

               - $65.00 First Week's Tuition
